Targeted Case Management (TCM) focuses on each individual’s strengths and abilities. TCM assists individuals by helping them identify those strengths, linking them to resources in the community, and helping to identify and overcome any barrier that may hinder them from succeeding in a goal. Individuals who are eligible to receive TCM services must meet the medical criteria and have Kentucky Medicaid or Kentucky MCO as their insurance.
Comprehensive Community Support Services (CCSS) are services necessary to allow recipients
with a primary diagnosis of a mental health disorder or co-occurring disorder to live with
maximum independence in the community.
CCSS are services to ensure successful community living through the utilization of skills training
and must be identified on the recipient’s plan of care. CCSS may be provided to an individual
recipient and are not group services.

We accept most insurances, please review the list below. Services may be covered fully or partially by your health insurance. You may be responsible for a copay depending on insurance coverage. You can easily check your own insurance benefits for counseling. Simply call the customer service number on your card and ask them for your “outpatient mental health benefits.” You will want to know your coverage for in-network providers and out-of-network providers.
